To request an individual or small group health insurance quote from an agent, please do one of the following:  

  1. Phone us at (408) 885-9170 or email us to request an appointment.

  2. For small group quotes, download and complete our Quote Request & Employee Census.  Please be sure to complete both pages (first tab is the Quote Request; second tab is the Employee Census) of the spreadsheet.  Save it to your computer, and submit the completed form to us by email.  This will tell us what quotations you would like us to secure from the marketplace.
